Preventing Avoidable Readmissions Together (PART)

More than 200 health care and community organizations have been a part of Preventing Avoidable Readmissions Together (PART), a South Carolina initiative aimed at reducing preventable readmissions. Since its implementation, targeted readmissions have been reduced by 15%, saving $14 million is associated healthcare costs.
