The U.S. Department of Defense Patient Safety Program and the Association of periOperative Registered Nurses created this toolkit to improve and standardize care coordination and communication among healthcare professionals throughout the handoff process (specific to perioperative settings). The toolkit, which includes an executive summary, talking points, patient hand-off tools, policy guidance and other resources, is based on the Department of Defense Patient Safety Program TeamSTEPPS initiative. These resources are most appropriate for a healthcare professional or administrator user.
