Sutter Health’s AIM Engages Patients, Saving Millions in Chronic Care Costs

This article outlines Sutter Health's promising adoption of advanced illness management (AIM). AIM is an approach that seeks to maximize patient outcomes by providing the appropriate curative and palliative supports well before a patient's condition is considered terminal. Essential elements of this approach include 1) a multidisciplinary team helmed by a primary care provider that supports the patient no matter their location (e.g., hospital, outpatient, home), 2) telehealth support, and 3) nurse liaisons that serve as advocates for patient care and familiar sources of support. The pillars of advanced illness management, Sutter Health's staff training approach, patient engagement tools and the organization's transition to consistent program documentation are all addressed in the article. Bottom line financial outcomes are also discussed; the program is on track to save the health system $29 million in Medicare costs.
