The Reducing Avoidable Readmissions Effectively (RARE) Campaign

This Minnesota Hospital Association toolkit identifies five elements as essential to effective readmissions reduction: 1) comprehensive discharge planning, 2) medication management, 3) patient and family engagement, 4) transition care support and 5) transition communications. Included on the site are a variety of tools and materials to this end, including a model gap analysis to determine areas for improvement, staff and patient education materials and checklists, discharge flow materials and additional recommended resources. The authors report exceeding their initial readmissions reduction goals; in the last quarter of 2013, participating organizations reduced their readmissions rate by 19 percent.
