This article discusses the ways in which pharmacists can play a central role in reducing hospital readmissions, including medication reconciliation; standardizing the discharge process; engaging and educating both patients and family members; post-discharge follow up in the form of telehealth; and initiating the discharge process well before the patient leaves the hospital.
