This case study outlines the work of the Everett Clinic's care management program for patients with COPD. Essential elements of their approach included refining clinical guidelines based upon the current GOLD guidelines; in addition, the team aimed to improve COPD diagnostic accuracy, implement a COPD registry, educate all relevant staff on their new approach, and monitor patient behavioral health regularly. Spirometry use increased from 16% to 40% over the course of implementation; in addition, the group reports that follow-up agreements established between hospitalists and pulmonologists are expected to impact their hospitalization rates in the longer term.
