This case study details a HealthCare Partners Institute for Applied Research and Education/HealthCare Partners Affiliates Medical Group telehealth initiative aimed at improving quality of life and reducing hospitalizations for patients with COPD. Specifically, the program added interactive voice technology (IVR) to in-person and telephone support, patient education and individualized action plans; the IVR calls used a "red, green, yellow" COPD symptom status categorization for consistency in communication and appropriateness of response. Results include improved clinical capacity, particularly for nursing staff.
