This case study outlines the efforts of Harvard Vanguard Associates' COPD care program, in which 600 of its patients were enrolled. This is an initiative that employed the use of multiple elements to improving COPD care, including proactive spirometry screenings; patient education; smoking cessation support; individualized management plans; follow up within seven days of hospital discharge; medication reconciliation; and electronic medical record transmissions to primary care providers. This case study includes information on involvement of organizational leadership as well as results and lessons learned; sample program forms are also included. This resource will likely be most helpful to front-line healthcare providers and health systems administrators.
