This PDF presents the University of Michigan Faculty Group Practice Quality Improvement Project case study started in the fall of 2010. The initiative aims to improve COPD care, health-related quality of life and decrease hospitalizations and emergency department visits. Essential components of this approach include the formation of multidisciplinary team to address patient care both during and post-hospitalization, targeted patient education, a primary care spirometry project and the tracking and documentation of quality improvement measures. Results of the effort and lessons learned are included; this document will be most helpful for healthcare providers and health systems administrators.
