This article details the Carolinas HealthCare population health management approach to maximizing care outcomes for its COPD patients. Their proactive approach involved prevention, early screening, timely diagnoses, appropriate use of resources, evidence-based care, chronic care management and palliative care. Care was delivered by a multi-disciplinary team, including respiratory therapists, family, palliative care and emergency medicine physicians and hospitalists and included patient education, telehealth and the incorporation of community resources.
