This article, appropriate for all PRAXIS users, shows that in Year 4 of the Hospital Readmissions Reduction Program (HRRP), 2,592 US hospitals will incur penalties (506 hospitals will be penalized 1% or more of their total Medicare revenue). While readmissions rates are declining, the data show that one of five Medicare patients is readmitted to the hospital within 30 days. Year 4 state data are included in the article; CSV files of both state and hospital data are available for download at the resource URL.
