Geisinger's Embedded Nurses Improve Transitions

Geisinger Health Plan's Health Navigator Program leverages the partnership between the payer and primary care physicians, putting nurse care managers on site with primary care in a medical home model. These care managers focus proactively on transitions of care, home safety and patient-empowering self-management action plans. Geisinger reports an approximate 20 percent reduction in readmissions for those enrolled in the program.
