This patient received a COPD diagnosis over 15 years ago and was only now hospitalized. She had smoked since she was nine years of age and is now 74. She hasn't smoked since she was hospitalized last time approximately 3 1/2 weeks ago. She has had Thoracentesis twice in that time and it was successful each time. She is on 2-3 for the Oxygen and has Bi-Pap at night. Her Bi-Pap is set to 12 in and 4 out. In her situation, what would you all recommend I recommend

What should I recommend in this patient's condition?
