I am trying to find out whether or not I have emphysema. I am a 71 y/o former smoker quitting about 15 yrs ago and then vaped for awhile cutting down to 0 nicotine before stopping completely a few yrs ago. This year when checking for lower Medigap rates, I was denied for a lower premium by a different company. I found out it was because a radiologist had put emphysema on my low dose Lung Cancer screen which I do every year. I was shocked. I thought it was because I had been given Albuterol by my internet when I had COVID. He also ordered a chest X-ray at that time which was normal. As result I requested a PFT to determine what I actually have going on. Findings were FEV1 2.76L, 120% predicted FVC 3.75L, 124% predicted; FEV1/FVC ratio 73%. Post bronchodilator there was no significant response. Lung volumes by body box show evidence of air trapping with TLC of 87% RV 273%, FRC 185% predicted. If fusion capacity corrected for hemoglobin is normal at 101% predicted inspiratory and expiratory. Flow loops are normal. his summary was there was no evidence of airflow obstruction and secondly evidence of hyper inflation without obstruction cannot rule out, emphysema based on this pulmonary function test. Keep in mind that I have had one chest x-ray and three low-dose CT scans none of which showed hyper inflation. Although the scans did mention mild emphysema. How can I have everything be so normal on the PFT and then have a reserve volume of 273%? And why do none of the cat scans were x-rays show hyperinflation? I just don’t know what to make of all this information. Obviously, this has a significant impact on whether or not I can annually check for lower insurance rates since insurance companies seem to automatically deny you based on respiratory diseas.
