A Comprehensive Care Management Program to Prevent Chronic Obstructive Pulmonary Disease Hospitalizations: A Randomized, Controlled Trial

This was a randomized controlled trial comparing a comprehensive care management program that included patient education visits, an action plan and telephone follow up with standard of care.
