This toolkit, most relevant to administrators and healthcare professionals, was developed by Boston University Medical Center with a contract from the Agency for Healthcare Research and Quality to assist hospitals in replicating RED, an initiative designed to improve the hospital discharge process and reduce readmissions. Demonstrated to both reduce readmissions and post-discharge emergency department visits, the program has 12 central components, ranging from ensuring language assistance for the patient when necessary to telephone reinforcement of the discharge plan.
