Medication reconciliation, the process by which a healthcare professional compares a patient’s current medication against all standing orders to ensure accuracy and appropriateness, is intended to decrease medication errors and patient harm. This toolkit can also be used to identify and respond to any gaps in an existing medication reconciliation process.
