Reducing the Readmission Burden of COPD: A Focused Review of Recent Interventions

This review article analyzed 107 research studies published from 2010 to 2014 to identify trends in patient risk factors and determine the most effective interventions in longitudinal, acute and post-discharge care. Socioeconomic factors shown to be predictive of readmissions; combination inhaled corticosteroid (ICS)-long-acting beta-agonist (LABA), ipratropium/tiotropium, antibiotic and corticosteroid therapies; care coordination; emergency department care; pulmonary rehabilitation; medication reconciliation; telehealth; nurse-led programs and integrated care programs are all discussed.
