Reducing Hospital Readmissions

This Center for Healthcare Quality & Payment Reform article offers three steps for reducing all cause readmissions: 1) using data to identify trends in your organization's readmissions, 2) adopting an approach that improves care across the entire care continuum and 3) using payment approaches that support your readmissions reductions efforts.
