Resource Type: White Papers/Reports

The Community-based Care Transitions Program (CCTP) was created under the Affordable Care Act to evaluate models of care transition for high-risk Medicare beneficiaries from hospitals to post-acute care settings. Of primary interest to the PRAXIS user will be the first annual program evaluation report (2014) of the initiative; the 111-page report includes background on the CCTP, a description of the 47 national CCTP sites, details on program implementation and results related to 30-day readmissions, emergency department visits and hospital observation service use.

The CCTP First and Final Evaluation Reports (PDF) can be located in the list of resources at the bottom of the Centers for Medicare & Medicaid Services reference page: http://innovation.cms.gov/initiatives/CCTP/?itemID=CMS1239313.
