Of primary interest to the PRAXIS user will be the first annual program evaluation report (2014) of the initiative; the 111-page report includes background on the CCTP, a description of the 47 national CCTP sites, details on program implementation and results related to 30-day readmissions, emergency department visits and hospital observation service use.
