Return to all articles Return to previous page STate Action on Avoidable Readmissions (STAAR) Resource Type: Presentations Toolkits 0 Comments STate Action on Avoidable Readmissions (STAAR) was a four-year regional Institute for Healthcare Improvement Initiative that aimed to reduce preventable readmissions through coordination among patient, family, community resources, hospitals and providers. The initiative focused efforts in two major areas: 1) improving transitions of care by creating a multi-disciplinary learning collaborative and 2) working with state leadership to effect change. This toolkit includes both high-level and detailed guidance on recommended areas for process improvement (e.g., patient-level assessment, timely post-discharge follow up) as well as examples of process and outcomes measures used; a robust materials section also includes initiative tools, presentations and issue briefs. This toolkit will likely be of most interest to health systems administrators, providers and policy makers.
