How-to Guide: Improving Transitions from the Hospital to Community Settings to Reduce Avoidable Rehospitalizations

This Institute for Healthcare Improvement (IHI) toolkit was developed to help hospitals and communities ensure optimal care transitions after hospital discharge. The toolkit includes recommendations for process improvements; an overview of recommended infrastructure changes and strategy central to successful implementation; case studies of organizations that have successfully implemented the toolkit and reduced preventable readmissions; and tools and resources to guide implementation and measure success.
