Deb McGowan Predictive analytics can give the multidisciplinary team the information they need to risk stratify patients who might be at high risk for readmissions. That information can facilitate the daily rounding process with critical information and help staff deploy their resources to the right high risk patients. It also helps to develop a personalized discharge plan and the team can begin working with the patients and their family on a plan that is right for them right from admission. Reply
