Another follow-up to topic with the same name.
I sent the below suggestion for another drug choice, that should not have the concerning side-effects of Spiriva even if in the same class, per research by Gemini. The doctor has prescribed the first drug Tudorza Pressair.
Also, read the patient reviews on Drugs.com - very happy patients for the most part:

To Doctor: I am reading about the following two drugs, one you mentioned. What do you think?
Proposed Alternatives (LAMA Class) - Both options below are in the same class as Spiriva but have different absorption profiles that may be safer for heart and brain sensitivities.
1. Aclidinium (Tudorza Pressair) -
Mechanism: Rapidly broken down by enzymes in the blood almost instantly. -
Brain Safety: Lowest risk of disorientation because very little active drug reaches the brain. -
Heart Safety: Proven cardiac safety in long-term studies (ASCENT-COPD); no increased AFib risk.
Dosing: 2 puffs per day (Morning/Evening).
2. Glycopyrrolate (Seebri) -
Mechanism: A "bulky" molecule that is chemically blocked from entering the brain easily.
Brain Safety: Low risk of "brain fog" or dizziness.
Heart Safety: Low risk, though can occasionally cause a fast heart rate.
Dosing: 2 puffs per day.
