This brief case study outlines Rutland Regional Medical Center's COPD Readmissions Reduction Project. Essential elements of their program include implementation of improved transitions of care, use of a standardized patient journal at all points of care, follow-up telephone calls one to three days post-discharge and clear patient medication education. Since program inception, when the COPD readmissions rate was 20%, rates have steadily decreased; Rutland Regional reports 2015 readmissions rates at 5.4%.
