In Pulmonary Rehab patients learn how to breathe easier and improve their health and well being. But too often after graduation they slip back into inactivity, poor COPD management and repeated exacerbations, sometimes leading to hospital readmissions.
The On Track with COPD Ongoing Support Program is a follow-up program helping PEP Pulmonary Rehab graduates continue in effective COPD self-management and maintain benefits gained during rehabilitation. Even for patients who participate in a Phase III maintenance program, On Track may be of benefit by providing an additional layer of connection and support.
On Track is optional and is offered by the C.O.P.D. (Call Our Patients Direct) Information Line. The information line is staffed by individuals directly impacted by COPD and well-trained in working with On Track.
As a participant in On Track a PEP Pulmonary Rehab graduate receives a phone call each month for up to 24 months from an Information Line Coach with support, motivation and accountability. The goal of this ongoing engagement is to help graduates continue, long-term, with the gains they made in Pulmonary Rehab.
A PEP annual report is provided to participating PR Centers, COPD Foundation partner organizations, and published in the COPD Digest and/or Lung Health Professional magazine.
All On Track activities are HIPAA compliant.
